
How To Apply For Change of Name in Passport?
To change your name in the Passport, you have to apply for “Re-issue” of Passport. To check the complete list of documents to be submitted along with the application form, please click on “Documents Advisor” link on Home page.
Steps To Apply For Reissue Passport
1. Register through the Passport Seva Online Portal. (Click on “Register” link on the Home Page).
2. Login to the Passport Seva Online Portal with the Login ID created in Step 1
3. Click “Apply for Fresh Passport/Re-issue of Passport” link. While applying under Fresh Issuance category – please make sure you never held a Passport of the applied category (i.e. Ordinary Passport, Diplomatic Passport or Official Passport) in the past. For instance, if you ever held an Ordinary Passport (Deep Blue colour) in the past but never held any Diplomatic (maroon colour) or Official (white colour) Passport – you need to apply under Reissue category if applying for an Ordinary Passport while choose the Fresh category if applying for a Diplomatic or Official Passport. Similarly, if you have/held a Diplomatic (or Official) Passport but do not have/held an Ordinary Passport – you need to apply for Ordinary Passport under Fresh category while choose the Reissue category if applying for Diplomatic (or Official) Passport.
4. Fill in the required details in the form and submit.
5. Click the “Pay and Schedule Appointment” link on the “View Saved/Submitted Applications” screen to schedule an appointment. Online Payment has been made mandatory for booking appointments at all PSK/POPSK/PO.
Online Payment can be made using any one of the following modes:
• Credit/Debit Card (MasterCard, Visa and RuPay)
• Internet Banking (State Bank of India (SBI) Associate Banks and Other Banks)
• UPI
6. Click the “Print Application Receipt” link to print the application receipt containing Application Reference Number (ARN)/Appointment Number.
Note:
Carrying printout of Application Receipt is no longer required. An SMS with your appointment details is also accepted as proof of appointment during your visit to Passport Office.
7. Visit the Passport Seva Kendra (PSK)/Regional Passport Office (RPO) where appointment has been booked, along with original documents.
Note:
• Only emergency/medical cases and preapproved categories may visit Passport Seva Kendra without appointment. Service will be provided at the discretion of Passport Seva Kendra in-charge/Passport Officer.
• In case of minor applicants (below 4 years of age), carry the recent Passport size photograph (4.5 X 3.5 cm) with white background.
• Resubmission of application form is required, in case the applicant does not visit the Passport Seva Kendra within 90 days from the online form submission.

There is a mistake in the spelling of my name in the Passport. What can I do?
Applicants are requested to take utmost care when applying online with the view to ensure that the Passports are delivered without any typographical errors since applicants have themselves fed the data. Also, every applicant is given opportunity to verify particulars at Counter-A of PSK/POPSK. However, if you find any mistake/error in the particulars printed in the Passport booklet as per the application form submitted, please return the Passport for necessary rectification at Regional Passport Office concerned. If the rectification is approved by the competent authority a new booklet with new Passport number would be issued. Any additional fees required to be paid would depend solely on merit of the case. In certain cases, applicant can be advised to apply for re-issue of Passport too.
