Steps: • Visit Digital Gujarat Portal • Register with Government of Gujarat Online Portal and get access to Services • This Service is available in both Languages English & Gujarati. • Photo uploading for all applicants is mandatory • Application will be considered complete only when all above these sections are completed. • All fields marked with *(star) are mandatory fields. • In case of any wrong/misleading information provided in application shall lead to rejection of the application by Department Authorities. • All required documents must be carefully scanned as per instructions given. Size limit should be mentioned. • The unreadable uploaded documents will allow competent authority to reject student application. • If your application is returned for change or to fill incomplete details, kindly submit it within 14 days of return. If fail to submit within 14 days application will be disposed with rejection. Application fee will not be refunded.

Extract of the Primary School Admission Register (Oldest Record Is Preferable): • Extract of the Primary School Admission Register of an applicant • Extract of the Primary School Admission register in respect of an applicant’s father • Extract of the Primary School Admission register in respect of an applicant’s grand-father • Extract of the Primary School Admission register in respect of an applicant’s great grand-father
Extract of the Birth Register (Oldest Record Is Preferable): • Extract of the birth register of an applicant • Extract of the birth register in respect of an applicant’s father • Extract of the birth register in respect of an applicant’s grand-father • Extract of the birth register in respect of an applicant’s great grand-father • Extract of the birth register in respect of an applicant’s any elderly relatives from paternal side
Extract of the Primary School leaving certificate: • Primary School leaving Certificate of an applicant • Primary School leaving Certificate in respect of an applicant’s father • Primary School leaving Certificate in respect of an applicant’s grand-father • Extract of the Primary School leaving Certificate in respect of an applicant’s great grand-father
Caste Proof (Any One): • True Copy of Caste Certificate in respect of an applicant’s father • True Copy of Caste Certificate in respect of an applicant’s Brother • True Copy of Caste Certificate in respect of an applicant’s Sister
Residence Proof Attachment (Any One): • Ration Card • True Copy of Electricity Bill. • True Copy of House Tax Bill • Tax receipt of residence • Extract from Aakaranee register
Validity Certificate (Any One): • Validity Certificate granted by the Scrutiny Committee of the applicant’s father • Validity Certificate granted by the Scrutiny Committee of the applicant’s real uncle • Validity Certificate granted by the Scrutiny Committee of the applicant’s any other elderly relative from paternal side
Proof Needed In Service Attachment: • Affidavit in Form A-1 • Family Tree affirmed by Talati (starting from great grand-father) • An extract of service record (book) mentioning tribe of the applicant’s father or blood relatives. • Any Other Documentary evidence in regard to the Scheduled Tribe and place of permanent residence prior to the date of notification of such Scheduled Tribe order
